Consider Sarah, newly diagnosed with psoriatic arthritis. Below is what a year inside the Resolyst loop looks like for her: the everyday quiet of continuous monitoring, the two moments it mattered most, and the network that surrounded her when it did.
Diagnosis & onboarding
Sarah's rheumatologist diagnoses psoriatic arthritis and starts her on apremilast. Before she leaves the clinic, she's onboarded to the Resolyst continuous care loop: a fractional MDT of specialist nurses, pharmacists and allied health professionals introduced as part of her care team.
Plantar fasciitis, handled
Around month 4, Sarah develops heel pain when walking. The MDT arranges an MRI of her ankle and foot, which confirms a tear in the plantar fascia. A specialist podiatrist consultation produces custom orthotics, all coordinated within the continuous care loop without requiring a visit to the rheumatology clinic.
Joint flare, resolved fast
Sarah wakes up with an acutely swollen, painful joint. She messages her Resolyst team. A specialist nurse reviews her case, escalates to her consultant rheumatologist for asynchronous review, and goes back to Sarah within a few hours with an anti-inflammatory regimen. No clinic visit needed. The specialist nurse checks in asynchronously each day for the following week to ensure no escalation to clinic is required for further investigation and treatment.
The case becomes complex
Six weeks later: acute bloody diarrhoea. The MDT escalates to her consultant rheumatologist, who recommends a colonoscopy. The Resolyst team navigate Sarah to a top gastroenterologist for the procedure the following day. Meanwhile, her consultant rheumatologist escalates her case to the Resolyst HLA-B27 International Expert Panel. The panel discusses her systemic therapy and recommends biologics.
Navigated & followed up
Sarah is diagnosed with acute colitis and started on treatment. For the next two weeks she's checked in on asynchronously every day by her specialist nurse as the colitis resolves. They then have a call to discuss the opinion received from the Resolyst International Expert Panel and schedule an in-person appointment at the rheumatology clinic once her acute symptoms have resolved fully. The loop continues.
